Fidelity & Guaranty Life is offering electronic tax statements for your convenience. You will now see an option on SalesLink to enroll in Electronic Tax Statements. By selecting this option, you will be authorizing FGL to furnish your tax statements to you electronically through a secured website.

• A faster, easier way to obtain corrections in your tax documents
• No more waiting for the paper documents to come in the mail
• Less phone calls trying to track down your tax statements

1. For those of you who have not yet made an election, you will be prompted to verify your details once logged into SalesLink.
   
2. Upon completion of the verification process, the enrollment screen will appear:
   
   At this point, you will need to make the election to either opt-in or opt-out.
   
3. Once you have completed your election, you will receive confirmation in the mail of your election and then be redirected to the home page.

• Access the internet and the Fidelity & Guaranty Life website
• Provide the Company with a current, working, personal e-mail address
• Have Adobe software to download, view, and print your statements
• Print or retain a statement for your records

Your authorization to receive Electronic Tax Statements now will apply every year going forward unless you change your election. You must either “Opt In” to receive Electronic Tax Statements or “Opt Out” to continue to receive paper statements.

If you previously "Opted Out," you may change your election. In order to do so, please click on Preferences, Subscriptions and 1099 Election in SalesLink to see the enrollment screen and change your election.

Annual statements will not be available until January 31st of the following year and will no longer be available on the website after October 15th of each year.

Incorporated, Corporations or agents under Internal Revenue Service federal threshold limits will NOT be able to access tax statements electronically.

Incorporated agents or agents whose commissions are under federal threshold limits are generally not reportable and will not receive a tax statement, be it paper or electronic.
